Elementary Differential Equations, 11th edition, by Boyce, DiPrima, & Meade; John Wiley & Sons Inc.

A minimum grade of C in either Math 220 or Math 230 and a minimum grade of C in either Math 273 or Math 283. (Math 220 or Math 230 may be taken concurrently.)
