$^{b}$ Submit completed scheduling form (including approved examination date, hour and place) to the Graduate School Office at least 10 working days prior to the examination date.

All forms may be picked up in the Graduate School Office, or downloaded via the web.

All program changes should be submitted on program change forms. Committee changes should be submitted to the Associate Dean of the Graduate School on forms provided and signed by the Chair of the major department and the Chair of the committee.

ENROLLMENT:

Students are responsible for satisfying all enrollment requirements stated in the Graduate School Policies and Procedures Manual, and in §14 of this handbook.
